Social Worker - Hospice Care

Key Responsibilities:

  • Conduct initial psychosocial evaluations of patients and their families to assess needs and develop appropriate care plans
  • Provide ongoing psychosocial counseling to support emotional well-being and coping strategies
  • Deliver direct casework services, including patient and family education, advocacy, and resource coordination
  • Assist with discharge planning to ensure smooth transitions and continued support post-hospitalization
  • Offer bereavement services to families experiencing loss, providing comfort and guidance
  • Engage in community education, outreach, and referral activities to promote awareness and access to hospice services
  • Collaborate with the interdisciplinary team to develop and implement individualized plans of care

Requirements
  • EDUCATION, TRAINING AND EXPERIENCE: Masters' degree in Social Work from an accredited educational institution. LMSW or LCSW preferred. Incumbent will have one year experience in individual, family and group counseling, preferably in a healthcare or hospice setting. MSW candidates will have 6 months to obtain LMSW.
  • CERTIFICATION/LICENSURE: Possess appropriate certificate / license to practice as a Licensed healthcare professional in the State of New York. Certification in Hospice & Palliative Care is preferred if offered for discipline.
  • Valid NYS driver's license required

Salary Description
$31 - $37 / hour
