Referring To Hospice & Palliative Care

We believe our role is to serve as an augmentation of your services-providing your patients or loved ones with specialized care and support they need at the end of life. We offer experience and expertise to relieve pain and symptoms, as well as emotional and spiritual support to enhance quality of life.

We know it can be difficult to have a discussion about end-of-life options. We can assist you by offering your patient or loved one pain and symptom management, improving their quality of life, and providing emotional and spiritual support to help when a cure is no longer an option.

We're available to answer your questions, make referral visits, and admit patients to hospice care. You can be confident that they'll receive the best care possible, so they can focus on what is most important to them.

Diagnosis

Hospice will benefit your patient or loved one by bringing them a support system in addition to the services you provide. We will create a patient care plan to address immediate needs, a long-term plan, and family support. Treatment options are important; we use pharmacological and non-pharmacological therapies, and allow patients to choose the right medication regimen for them.

We can offer alternatives to hospitalization, helping patients experiencing any end-stage illness, including, but not limited to:

  • Alzheimer's & dementia
  • Cancer
  • Cardiopulmonary disease
  • Lou Gehrig's disease
  • Liver disease
  • Renal disease
  • Stroke/coma
