Hospice LMSW

Hospice is seeking a compassionate, talented LMSW Social Worker that is eager to join an organization that focuses on the patient and family.

Hospice LMSW Social Workers enhance the lives of patients with life-limiting illnesses & their loved ones, during a time when compassionate care is needed most. You will provide supportive counseling services to help meet the patient & their families social & emotional needs in relation to their diagnosis. Social Workers provide 1-1 services in patient homes, nursing homes and other home-like residences.

Experience in homecare, long term care acute care setting preferred. LMSW required.
