Referring To Hospice & Palliative Care
We believe our role is to serve as an augmentation of your services—providing your patients and their families with specialized care and support they need at the end of life. We offer experience and expertise to relieve pain and symptoms, as well as emotional and spiritual support to enhance quality of life.
Our Medical Directors are Dr. Randolph Snow, Dr. Joseph Booth and Dr. Scott Brehaut.
We know it can be difficult to have discussions with patients and families about end-of-life options. We can assist you by offering your patients pain and symptom management, improving their quality of life, and providing emotional and spiritual support to help patients and families when a cure is no longer an option. We are also here to support you and your staff.
We’re available to answer your questions, make referral visits, and admit your patients to hospice care. You can be confident that they’ll receive the best care possible, so they can focus on what is most important to them.
Please call our Admissions Office at 315-735-6484 or 1-800-317-5661 to make a referral.
Hospice will benefit your patients by bringing them a support system in addition to the services you provide. We will create a patient care plan to address immediate needs, a long-term plan, and family support. Treatment options are important; we use pharmacological and non-pharmacological therapies, and allow patients to choose the right medication regimen for them.
We can offer alternatives to hospitalization, helping patients experiencing any end-stage illness, including, but not limited to:
- Alzheimer’s & dementia
- Cardiopulmonary disease
- Lou Gehrig’s disease
- Liver disease
- Renal disease
Referring professionals may receive a copy of our disease-specific guidelines by contacting our Admissions Department at 315-735-6484 or 1-800-317-5661.
